kids-hpNorthern Queensland Primary Health Network (NQPHN) is working to make health better for all people in North Queensland. We have a focus on improving health for populations that are most at risk, such as Aboriginal and Torres Strait Islander people and older Australians. We are actively engaging people in communities through our Community Advisory Groups and we also welcome feedback on health services and issues.

AGED CARE | Creating better health care through innovation: GP video consultation with aged care

Northern Queensland PHN's Better Health Care Connections (BHCC) pilot project aims to improve the quality of health care for aged care recipients in residential aged care facilities (RACF) who have complex care needs.

The project aims to implement a model of care where GPs provide video consultations, in addition to face-to-face contact, to patients in RACF and incentivise them for the service.

NQPHN BHCC Coordinator Elyse Donoghue recently spent time with the Western NSW PHN, where the project has been a huge success.

The project in Broken Hill has completed over 600 GP video consultations, with six GPs and three RACFs on board.

Along with developing great cross-PHN networks, Elyse has brought back innovative ideas of how to increase and expand the project in our region.

To date, three RACFs are involved in the project from Townsville and Ingham, servicing approximately 300 residents.

The project is currently expanding and seeking additional RACFs and GPs from within the Townsville Hospital and Health Service region to be involved in the project.
